How to collect & return your sample
Before you get started, it's important to avoid eating, drinking (including water), smoking, or chewing gum for 60 minutes.
Collect your sample:
1.
Open the package and remove the swab without touching the sponge tip.
2.
Position the swab inside your left cheek where saliva gathers, between the inner cheek and bottom jawline. Swipe back and forth 10 times. Repeat on your right cheek.
3.
Hold the tube upright to prevent the liquid inside from spilling. Unscrew the blue cap without touching the sponge.
4.
Insert the sponge into the tube, and close it tightly. Gently mix 15 times.
5.
Write your legal first and last name (no middle names), date of birth (Month/Day/Year) and date you collected the sample (Month/Day/Year) on the label provided. Use a pen (markers smudge).
6.
Leave the MRN field blank
7.
Place the label around your swab tube carefully, just below the closed lid.
8.
Log into members.eugenelabs.com and complete onboarding
Return your sample:
1.
Place your tube in the sealed bag and back into the smaller white box provided.
2.
Peel off the tape protector and seal the box closed.
3.
Return your sample in any yellow Australia Post box or post office.
